Psoriasis of the palms and soles may appear as confluent redness and scaling, discrete plaques, ill-defined scaly/fissured areas, or confluent plaques extending to the wrists and to the margins of the soles and heels. It not infrequently extends to involve the dorsal surfaces of the fingers and hands. It may sometimes be difficult to distinguish from hyperkeratotic eczema of the palms and soles and from palmoplantar pustulosis.
